This plan is designed to be affordable, but the main thing is that it is an in-patient (you're in a hospital) plan along with emergency room care.

Plan pays 80% after the $1000 deductible for the following:
- Professional services (in-patient) and hospital services ($500 inpatient copay), surgery, anesthesia, radiation therapy, in-hospital doctor visits, diagnostic x-ray and lab
- Home Health Care - 100 visits per year

Here is a plan that adds outpatient benefits!
In-Network Benefits:
just a $30 office copay for a doctor visit with no deductible to be met
Deductible choices go from $1000 all the way up to $10,000
Has unlimited maximum benefits - you can't outspend this plan

Immunizations to age 5
The Blue Card Program - use the Blue Cross and Blue Shield plans in all the

Plan Three - Alliance Choice Plan
The key here is GREAT BENEFITS! You can get maternity as an option and prescriptions are covered with just a copay. But try not to get pregnant until you know you can deliver 12 months after the policy starts because the maternity coverage starts after 12 months.
Here's how the plan works with network providers:
Choice of $0 to $10,000 deductibles
Choice of the plan paying 80%, 90%, or 100% after the deductible
$20 office copay to a general care physician
